He was found to have elevated pancreatic enzymes, and was discharged with the diagnosis of acute pancreatitis. He returned to the ED twice thereafter for persistent abdominal pain, and was eventually admitted. Core needle biopsy performed under endoscopic ultrasonographic guidance of the pancreas showed diffuse large B cell lymphoma. He was started on cyclophosphamide, doxorubicin, vincristine, and prednisone, plus rituximab (R-CHOP), and was subsequently discharged. He completed six courses of chemotherapy with no evidence of recurrence on outpatient imaging.⁠⁠Pathology: The core biopsy of the pancreas showed diffuse large B-cell lymphoma, germinal center origin, with Immunohistochemistry stains positive for CD20, CD10, BCL-6, and with a Ki-67 of 90%.⁠⁠Physical exam: n/a⁠⁠Figure legend: Axial IV contrast-enhanced CT images acquired in the portal venous phase show a homogeneous low attenuation mass centered with the pancreatic head, replacing normal parenchymal architecture (red arrow). Similar findings are noted in the tail of the pancreas (red arrow).⁠⁠Timecode: 6 Baseline⁠⁠Other diagnostic testing: A bone marrow biopsy showed no lymphoproliferative process. His white blood cell count upon presentation was normal, 7000/mL.
